• Master Carrier/Broker Agreement, signed

  Insurance Certificate Naming Certified Freight Logistics, Inc. as certificate holder and an endorsement listing same as additional insured with the following coverages:

  • Truckers Auto Liability insurance ($1,000,000/CSL)
  • Cargo, broad form with reefer/breakdown ($100,000)
  • General Liability Insurance ($1,000,000) (optional)
  • Trailer Interchange Coverage ($75,000) (if pulling our trailers)
  • Workers Compensation Insurance (if you have employees)
• Copy of your California Motor Carrier Permit (if princple office located in California)
• MC Number
• US DOT Number
• W-9 Form
• Compliance Agreement for California Air Resources Board Regulations
• Trailer Interchange Agreement (if you are pulling our trailers.)
